When does the “permanent bar” prevent immigrants from receiving a green card?

One of the most common reasons that otherwise law-abiding undocumented immigrants are unable to get a green card through their family members is because they have entered the U.S. multiple times without inspection and fell afoul of the "permanent bar" of INA 212(a)(9)(c). It doesn't matter if they are married to a U.S. citizen, or if they have raised a child who goes on to win the Medal of Honor and a Nobel Prize... they will still be unable to get a green card just because they left the U.S. and then entered again!
